If you use Windows, it's enough to have Windows Defender (or some popular alternative like Avast) on.
They scan files both when downloading and when running.
You will get a warning if the file is suspicious.

No way to be 100% sure with unofficial repacks. Scan with VirusTotal before running anything. Best practice: buy legit copies. If you must use repacks, stick to well-known trusted sources and sandbox first. Still a gamble though - you're executing code someone else repackaged without oversight.
